Disclaimer for School-Issued Device Use

As an educator provided with a device by Horizon Private School – Branch, I acknowledge and agree to the following terms and conditions:

1. Purpose of Use: The device is issued solely for professional use in carrying out my responsibilities as a teacher. It is intended to support my work related to curriculum delivery, student engagement, and communication with school stakeholders.

2. Ownership and Property: The device remains the property of Horizon Private School – Branch. I understand that I am responsible for its care and maintenance while it is in my possession.

3. Acceptable Use: I will use the device in accordance with the school’s Acceptable Use Policy and all relevant guidelines and procedures. Personal use of the device should be limited and must not interfere with professional responsibilities.

4. Security and Privacy: I will take all reasonable steps to protect the device from loss, theft, or damage. I will not install unauthorized software or make any modifications to the device. Any data or information stored on the device should be handled in a manner that respects privacy and confidentiality.

5. Return of Device: I agree to return the device upon request. The device should be returned in good working condition, subject to reasonable wear and tear.

6. Liability: I understand that I may be held liable for any loss, damage, or unauthorized use of the device resulting from negligence or violation of school policies.
